Throw is now behaving and has been let out of skill jail.

- added a manual GUI.

- also added a Patchouli manual. Get it at spawn or by crafting a book with two wooden swords if you lose it. Why two? Because dual wielding.

- coup de grace now replaces the expose damage bonus with its own bonus.

- widened the vertical parry angle.

- offhand attacks now require combat mode, so I can stop accidentally killing my pigs.
