Performance attribution bug fix.

Fixes:

  • Performance traces now filter top-addon attribution through the current loaded-addon state.
  • Unloaded addons are no longer named in top_threshold or top_last_tick_ms evidence.
  • Existing bounded performance tracing and conservative not-root-cause correlation are preserved.
  • Existing passive incident, privacy, sanitization, and user-controlled report workflows are unchanged.
