Performance attribution bug fix.
Fixes:
- Performance traces now filter top-addon attribution through the current loaded-addon state.
- Unloaded addons are no longer named in top_threshold or top_last_tick_ms evidence.
- Existing bounded performance tracing and conservative not-root-cause correlation are preserved.
- Existing passive incident, privacy, sanitization, and user-controlled report workflows are unchanged.
