v0.12.4 (2026-07-17) — map markers expire

Death skull pins now expire. They marked every unavenged death forever; now they show where the fight is, not where it once was — pins older than 30 minutes (configurable 5–240 under Options → Map pins) are swept off both maps once a minute. The death itself stays on the Deaths tab and can still be avenged; only the marker goes away. TomTom rally arrows expire too. The waypoint was only removed when the caller explicitly cancelled — if the rally simply ran out, the crazy arrow pointed at nothing forever. It now auto-removes after 30 minutes as a backstop (a cancel still clears it immediately).
