What does this Addon?

Its just plays the mario death sound if you die by any reason.

Volume of the the sound is bound to the "sound"-slider.

If you want to play your own sound, you could replace the "dead.mp3" in the Addon folder with your own mp3 file. Just make sure it's also named "dead.mp3".
