Operation: Ghost

Fixed: sentence-restructuring templates could get double-transformed by the word dictionary, producing broken grammar like "Rendezvous with quietly with X" (was "Meet quietly with X" run through the "meet" -> "rendezvous with" swap a second time). Reworded all affected templates so they no longer collide with dictionary trigger words.

Fixed: the briefing header tag and corner note weren't showing up when viewing quest details through the World Map's quest log specifically (they worked fine from the quest-giver dialog and Immersion). The header search now checks the Quest Log, World Map details panel, Immersion, and Gossip frames directly instead of only searching near the quest description text, and the corner note now anchors to whichever of those panels is actually visible instead of defaulting to one that might be hidden.
