Mute Buttons

Volume control
- Mouse wheel over an icon adjusts its volume (master for the speaker, music for the note). Step size is configurable, 5% by default.
- Hovering an icon opens a slider flyout showing the current level. It can sit left or right of the icon, horizontal or vertical, with its own scale and hide delay, or be turned off.
- Preferred volumes: set a master and music level in options, and the star on the flyout jumps back to it. A gold mark on the bar shows where it is.
- New slash commands: /mute vol, /mute musicvol, /mute pref, /mute setpref, /mute flyout, /mute flyscale, /mute tooltips.

Options
- Options panel reorganised into Buttons and Volume columns; flyout settings grey out when the flyout is off.
- Tooltips on the icons can be switched off, and they now anchor away from the flyout and the screen edge.
- Scale slider moves in 0.01 steps.
- "All sound" volume labels renamed to "master" to match Blizzard's sound panel.

Fixes
- Toggling the dark tile no longer shifts the icons.
- Flyout and tooltip no longer run off the bottom of the screen.
