- Instance toggles now fully respected:
- Open World / Battlegrounds / Raids / Dungeons apply the correct max debuffs per context.
- Extras (#4–#12) are **hidden** when a context is disabled (no leftover icons).
- Hooks now compute the correct per-context value on every aura update.
