Fixed
- Reminder no longer appears when using the dungeon teleport from a city — the teleport briefly passes through a transition zone that triggered the post-instance reminder incorrectly. Now only real dungeons (`party`) and raids (`raid`) set the instance flag.
- Replaced `reminderShownThisLogin` race condition with `isLoginPending` flag set by `PLAYER_LOGIN` — ensures the reminder timer only starts on a true login or relog, never on a dungeon teleport.
