A simple home mod

A simple /home mod. Adds 4 commands:

/sethome [name]: Sets a home point
/delhome [name]: Deletes a home point
/home [name]: Teleports to a previously set home point
/listhomes: Lists all your home points.

Homes are stored per dimension and per player. You can have multiple homes. If you don't give a custom name, the name 'home' is used.
