Better Music Toast

• Fixed: Songs from resource packs and music mods (for example Medieval Music) showed the name of the Minecraft song they replace. The display now shows the song's own name: from the pack's language file, from the title and artist stored in the music file, or else the name of the resource pack or mod.
• Changed: New songs that a resource pack adds now name the pack as their source.
• New: Shows the songs of Reactive Music, Music Triggers and Music Player, which play music with a player of their own. None of them is needed.
• New: Music discs from mods that name their song only in the "Now Playing" message (for example Net Music) show that name.
