To equip the backpack, simply equip the item in your off-hand!
To use the backpack, you will need to type /backpack with the item in your off-hand while crouching, and it will open (you only need the item if it is enabled in the settings).

Can I give free backpacks to everyone?
Just activate the /freebackpack command (the configuration guide should be below somewhere).
