Rewrite as lightweight hook into Travel module

Replace the standalone TeleModule that duplicated XIV_Databar's hearthstone logic with a minimal hook-based approach. It should reduce possible breaking changes from main addon. Enable customization of the command (e.g., changing to Porter).
