    v0.4.2: Multi-source tracking, encounter dedup, edit note improvements

    - Multi-source display: players show all sources (Group, Dungeon, etc.)
    - Source filter and stats now check all encounter sources
    - Enter saves / Escape cancels in Edit Note dialog
    - Fixed duplicate encounters from wipes and corpse runs (dedup by source+zone)
    - Fixed alert popup firing on member leave (session-based tracking)
