Handle ensemble transmog and add commands

Treat rewardType "ensemble" like "transmog" in ownership checks but use campaign.appearanceItemID for the PlayerHasTransmog lookup (ensembles consume the original item). Update a campaign entry to use rewardType="ensemble" and add appearanceItemID. Add a Commands table to the README and remove the duplicated commands section to clean up documentation.

Also made the addon scan in certain cases to check if it picks up any new items, no performance decrease.
