Version 1.2: Expand utility system, add advanced settings UI

- Adds 4 new utility buttons (Fire/Earth Elemental, Heroism/Lust, Shamanistic Rage) for a total of 7, all with per-utility show/hide and drag ordering.
- Utility layout now supports side/direction selection and independent horizontal/vertical spacing.
- Button bar layout is bounding-box based for flexible utility placement.
- Settings UI expanded to 7 tabs (General, Totems, Utilities, Fire, Earth, Water, Air); Utilities tab allows full utility customization.
- Cast macro order is now user-configurable and can be linked/unlinked from display order.
- More hot path optimizations to keep your CPU purring and used resources low.
