4.0 updated table of contents file so this addon wouldn't be considered "out of date".  No changes were needed for 4.0 compatibility.

1.2:  Added the spell "Banish" to the addon, to alert resto druids of warlocks casting it.

If a player begins casting a spell straight out of stealth or shadowmeld, it does not cause a combat log event to the player, thus causing the addon to not detect the cast.
