  Remove weird calculation that worsened Rage estimates near the Rage cap.
• refactor: reorder calculation of Rage gained with Violent Outburst
  Since we no longer double-count the rage gained from Shield Slam and
  Thunder Clap, move the calculation of the Rage gained with Violent
  Outburst into the direct variable for rage gained from those two
  abilities.
  Also rename from rage_gen to rage_gain for consistency.
