Added per-bar width/height for MH, OH, and Ranged.

extra attacks handled differently and will not reset the bar anymore which is more accurate to how the actual game works. and will increase accuracy during auto attacks after extra attack procs by very small amount.
On-next-swing abilities — HS/Cleave/Maul/Raptor seperated mechanically from slam in logs
Parry haste — floors at 20% of weapon speed instead of 0.001
