Johnny C. Lam:
    - Add new mapping from layout names to by-role layouts.
    This fixes the bug where the Grid frame wasn't updating after the groups
    were reorganized by roles when zoning into a new instance.
    
    Also add some debugging to UpdateGroups().
    - Hook GridLayout:ReloadLayout() to ensure the groups are accurate.
    Explicitly call UpdateGroups() to ensure the secure groups are accurate
    before rebuilding the layout in GridLayout.
    - Add README.md file for main description.
    - Remove unprintable characters from TOC file.
    - Initial import of GridLayoutByRole.
    GridLayoutByRole is a Grid layout option where the roster is grouped by
    role.
