DynamicMacros for PvP

!!! Major Update !!!

1. Pesky bug which caused addon sometimes to not update macros between solo shuffle rounds should be gone
2. @cursor has been added to protection list
3. From now on addon will be automatically triggered only in arena instances (2v2 / 3v3 / SoloShuffle) not in outside world
4. New command **/dmt ** to trigger addon functionality manually for testing purposes outside of arena or in case addon wont update your macros as it should.
5. Readme on github has been updated with info about /dmt command
