Updated ToC and version to 6.0.
Updated LFR and Flexible raid difficulty indexes that use instance chat when set to raid output.

Added checks for Versatility buffs to see if people need MotW.
Added primal tenacity to absorb tracker.
Removed Shroom tracker code since Wild Mushroom: Bloom has been removed from the game.
Removed Hibernate code.
Removed Nature's Grasp code.
Removed Innervate code.
Removed Might of Ursoc code.
Removed Symbiosis code.
Remove CCs from allcc_ids that no longer exist: Hibernate, Bind Elemental.

BUGFIX: Shroom Tracker now supports Glyph of the Sprouting Mushroom (glyph added in 5.4).
BUGFIX: Properly output to instance chat in scenarios when using RAID and PARTY chat (bug did not exist prior to 5.4).
