Fixes

• Same-spec players (e.g. two Frost Mages) were getting each other's names mixed up
• Realm name was appearing on player names during combat
• Meters stopped tracking if you died mid-fight — now continue until the whole group leaves combat
• The "break snap" dot was showing on all frames after every reload/login — now only appears on the frame that is actually snapped to another

New Features

• Refresh rate slider — control how fast the meters update (0.1–2.0 seconds)
• M+ run saved as a segment — when you finish a Mythic+ key, it's automatically saved as a named segment in the dropdown (e.g. "Darkflame Cleft +7")
