Trinket changes:
* Forgemaster's Insignia and Battering Talisman now tracked as RPPM trinkets
instead of stacking. This should make them work correctly in the cooldown tracker
DW Frost Necrotic Plague:
* Fixed missing Plague Leech
* Fixed Blood Tap triggering too soon/often
* Cleaned up Disease application code, removing possible error causing reference
to Unholy Blight and simplyfying code for application using Outbreak or Plague Strike
