    fix: Spell icons not showing when 'Show When' is set to 'Out of Combat' or 'Always

    - Fixed combat state not updating immediately when changing 'Show When' setting
    - Fixed addon not detecting combat state correctly on reload

    refactor: Hide icons when panels are open excluding when in preview mode
