    Fix "Spell not found on action bars" error on Blizzard default UI by
    rewriting AddStandardButton to use spell IDs directly from GetActionInfo

    mixup, adds missing slot fallback, and corrects DefFetch functions for
    DiabolicUI/AzeriteUI.
