Better Northern Sky TTS

Big update - thus version 4.0 - tested for over a week with 3 raid nights, looks good - but if anything, report bugs and/or rollback to 3.8.

What's new:
- Major refactor - should make the addon even more lighweight
- Added a excluded words functionality: now you can add words that will be ignored by TTS and not be read (i'm for axample ignoring 'power' and 'word' such that every time 'rebuff power word fortitude' would play i get 'rebuff fortitude' only)
- Added excluded words configuration window in the addon settings panel

What's to come:
- Add abbreviations functionality (replace multiple words with one, for example addon would want to say 'Invoke Yu'lon, the Jade Serpent' we could abbreviate the whole sequence of words to just 'Yu'lon') - you could achieve the same thing by ignoring every other words, but sometimes you want to hear 'invoke' in other context)
