Select Difficulty While In-Game

In order to access the EXTREME DIFFICULTY level in the game, you need to unlock it by default, based on my understanding.

Additionally, if you decide to adjust the difficulty settings while in a combat zone (like EXTREME to NORMAL), it's important to load a waypoint; this step is crucial for the game to accurately recognize the difficulty change you made. So enemies are scaled to their proper levels!
