It's technically not a preset, but a new, mostly seamless mesh. You can find it under Tattoo - Arm lower (right) section, this way it won't disappear from your sims with changing clothes like usual accessories and have more detalization unlike common face presets. Adult frame, unisex. You may want to download "No ears mod" preset from CmarNYC on MTS so you can hide original sim ears. Try to avoid hairstyles with shadow textures.
