These are set for NO RANDOM, which you can change in the package itself using Sims 4 Studio, and go into Categories, scroll to the bottom, and tick Allow For Random, then Apply To All Swatches and save (for your personal use). No Random means no sim will show up wearing the hat unless you put it on them.
