This world was made using the Five Nights at Freddy's: Management Wanted Mod, with it's addon the Surveillance Package.

This map is that fnaf location the showed up in the movie. It does have changes though, like the bathrooms being moved, and i am sorry if some things aren't accurite but i only had a few images to go off of.
