Change the totem file player/assets/minecraft/textures/item/totem_of_undying
to your skin, but change the file name to match the file name for it to work.
It doesn't require any mods; it works with vanilla Minecraft or with mods.
Place it on top of the texture packs for it to work and not change its model.
