Mystic Midnight: Requiem

A few recipe fixes.

Because of the way the modpack deals with dungeon loot, certain items that were normally supposed to be found in nether fortress chests (like Bewitchment saplings or the Codex) were near impossible to find.

So I added recipes for those items.
