Map worlds

Added

• Banner patterns, player head skins, sign text (with colour and glow ink), maps and other items in item frames, and paintings now appear in map worlds, just like on the server.
• Books on lecterns can be read: right-click a lectern with a book. Taking the book is not possible.
• /tp <x> <y> <z> (also with ~ for relative positions) and /tp <player> work in map worlds, including players in another world of the map. /teleport works the same way. On normal servers /tp is unchanged.

• What you see of banners, heads, signs, maps, items in frames, paintings and books depends on the map owner's privacy settings. Owners, co-owners and admins always see everything. Server owners need VoxelMap plugin 0.5.5 or newer for these to show up.
