Turbo for Distant Horizons

Added

• Config excluded_dimensions: dimensions listed there (by id, for example minecraft:the_end or a mod's dimension) get their distant terrain from the Distant Horizons generator, the rest still from this mod. Works in singleplayer, on servers and for clients that generate the distance themselves, and takes effect without restarting the game.

Removed

• The "DH Turbo: ... tiles" status line. It showed up on the screen even with the debug screen (F3) closed.
