Changes
- Fear damage reworked into random heart spasm attacks
- At 90%+ fear: rare sharp chest pain (~every 75s, 0.5–1.5 hearts)
- At 100% fear: random panic spasms (~every 10s, 0.5–1 heart)
- Removed continuous damage — feels like sudden panic attacks
