Synchronizing teams from FTBLib to scoreboards

This is a very simple server-side mod, which automatically creates and deletes scoreboard teams, when a FTBLib team is created or deleted. It also adds and removes players from scoreboard teams when a player is joining or leaving a FTBLib team.
