Report a block once and it's gone for good — removed from the world and permanently unplaceable, per save.
Report Station — a small utility mod that lets a world vote a block out of existence.
Craft it from eight iron ingots ringed around any planks, right-click it, drop in any block, and hit Report. That block is banned for the entire save: every existing instance is removed from the world, and any further attempt to place it is rejected server-side with a chat warning. The ban list is stored per world, so it survives restarts and never leaks into other saves.
