Description
Perelom — Body Damage
A detailed body injury and medical system for Minecraft 1.20.1 (Forge).
Forget the vanilla one-bar health system. With Perelom, every injury affects a specific part of your body, changes how you move and fight, causes bleeding or infection, and requires the right treatment at the right time.
A broken leg can force you to crawl. A bite can develop into a deadly infection. Heavy injuries can reduce your maximum health. Every wound matters.
⚔️ 22 Body Parts & Realistic Injuries
Your body is divided into 22 individual zones:
-
Head: HEAD_1–4
-
Torso: BODY_1–6
-
Arms: LEFT/RIGHT_ARM_1–3
-
Legs: LEFT/RIGHT_LEG_1–3
The affected body part depends on the damage source and your current pose.
-
Falling primarily damages the legs.
-
Zombie attacks can hit the torso.
-
Ranged attacks can strike the body part you aim at.
-
Armor actually matters: exposed body parts are more likely to be hit, while protected areas are much harder to bite.
Injury Types
| Injury | How it happens | Effects |
|---|---|---|
| Scratch | Light attacks, minor falls | Quickly treated with a plaster |
| Bite | Zombies, ghouls, wolves, spiders | Bleeding and potentially deadly infection |
| Open Wound | Severe bites or cuts | Causes continuous bleeding until bandaged |
| Fracture | Hard falls and heavy attacks | Pain, reduced mobility and maximum health |
🦵 Broken Legs
Fall damage isn't simply a number anymore.
A small fall may only cause a scratch. A medium fall can fracture a leg. A severe fall is very likely to cause a fracture, while an extreme fall of 15+ blocks can break both legs.
You can have up to 6 leg fractures at once. Each additional injury attempts to affect a body segment that hasn't already been broken.
The more fractures you have, the worse your mobility becomes.
-
1 broken segment: noticeable slowdown
-
3–4 fractures: forced crouching
-
5–6 fractures: crawling only
🩸 Injuries Affect Your Character
Injuries aren't just cosmetic.
❤️ Reduced Maximum Health
Severe fractures and open wounds temporarily reduce your maximum HP.
When you recover, your health returns gradually instead of instantly jumping back to full.
🦵 Movement
Broken legs progressively reduce your movement speed.
Severe leg damage can force you to crouch or crawl.
🗡️ Broken Arms
Arm fractures increase your weapon attack cooldown, meaning you have to fight more slowly.
😖 Pain
Leg fractures apply a pain effect. Jumping also causes damage, with the severity increasing as more fractures accumulate.
🩸 Bleeding
Open wounds continuously drain health until they are properly bandaged.
☣️ Bite Infection
A bite is much more than a simple injury.
If you don't treat it in time:
-
You have 10 minutes to bandage the bite.
-
If you miss that window, the infection becomes incurable.
-
The infection progresses over 3 in-game days (~1 real hour).
-
Symptoms become progressively worse:
-
Weakness
-
Slowness
-
Hunger
-
Blindness and near-total immobility
-
-
If the infection reaches its final stage, the bite kills you.
☠️ Death Has Consequences
Dying from an untreated infection is not the end of the story.
A zombie double appears at the location of your death and takes your entire inventory with it — including backpacks and external storage.
Your previous life won't simply disappear. You'll have to find what was left behind.
🩹 Health Window — Press Y
Press Y to open the dedicated health interface.
The GUI contains 22 medical slots, one for every body zone.
Each injury must be treated individually and with the correct item.
-
Bandages only work on wounds and bites.
-
Plasters only work on scratches.
-
Splints are required for fractures.
-
Incorrect items cannot be placed into an injury slot.
-
Applied medical items are consumed and remain attached to the wound.
-
Closing the GUI does not cancel treatment.
-
Treatment progress runs on the server and is saved with the player's data.
-
Progress is displayed directly in the medical slot with a green cross and progress bar.
-
Hover over the slot to see the exact treatment percentage.
Your treatment continues even after you close the health window or leave the world.
🧰 Medical Items
| Item | Treats | Recipe |
|---|---|---|
| Bandage | Wounds, bites and cuts | White Wool + String + White Wool → ×4 |
| Plaster | Scratches | 2×2 Paper → ×4 |
| Cloth Splint | Fractures | 4 Sticks + Wool → ×1 |
Bandages and plasters can also be crafted directly in the 2×2 inventory crafting grid.
🖼️ Interface & Visual Feedback
The health interface provides clear visual feedback about your condition.
-
Injury icons have slight random rotations, making identical injuries look different.
-
Orange highlights indicate light and medium injuries.
-
Red highlights indicate severe injuries.
-
Severe injuries include injuries with severity ≥ 60 or 2+ fractures in the same limb.
-
The GUI tells you which item is required to treat each injury.
-
Injury data is synchronized between the client and server and displayed correctly to other players.

❓ FAQ
Can I cancel treatment by removing the bandage?
No.
Medical items are consumed when applied and remain on the injury until treatment is complete.
Do injuries survive death?
Yes. This is enabled by default. A new life does not magically heal your injuries.
Can I treat a bite after the 10-minute window?
No.
This is an intentional gameplay mechanic. Once the infection becomes incurable, the bite will eventually kill you.
Can I change the balance?
Yes.
