Fixed

• Duplicate item names collapse for short CJK names too (铁锭 / 木棍), not only names of 3+ characters.
• Item markers without a namespace ({{item:stone}}) render as the item name instead of leaking raw braces.
• A tool that throws is reported (TOOL_ERROR in the trace) instead of counting as "not in this pack", so fallbacks still run.
• English replies no longer delete short Chinese item/block names.
• Empty item markers are no longer filled with the same icon repeatedly; an unterminated marker no longer breaks the rest of the reply.
• One failing JEI category no longer discards every recipe card for the item.
• A bad machine_requirements.json entry is skipped and logged instead of dropping the whole category.
• Only one ask runs at a time; a failed ask is reported once instead of twice.

Performance

• Pack index text cache capped at 24M chars; oversized pack files are logged instead of silently skipped; an empty-but-built index is no longer rebuilt on every ask.
• Guidebook index and item search stop rescanning every mod jar / the whole registry; a pack without Patchouli books caches that negative result once.
• Item index building is chunked, so it no longer blocks the render thread for seconds.

Tooling

• autotest status now carries verdict/expectedCaseCount, a case cannot be OK without evidence, and a case can no longer be judged from another item's trace.
