LOTR Improvements

option for lotr HP bars to show above entities instead of hearts
made bars show by default instead of hearts (looks cleaner and less cluttered)
option to auto disable existing lotr indicator (enabled by default)
fully removed yellow attack meter (neutral/passive entities will have the white attack indicator now)
fixed hp showing on horse a player is riding on and over hired entities
fixed attack indicator gl errors
fixed NEI still highlighting empty pouch slots when nothing was in the search bar
fixed a crash with some mounts when using mountinfo (some mounts don't have tamer info in them)
fixed health viewer and text scaling over distance
