Player feedback survey on the death screen

A new Give Feedback button on the death screen opens a short survey — a 0–10 rating plus an optional comment — and posts the response to Discord, into the player's own thread.

  • One new question per death; a question is never asked twice, and the button hides once a player has answered everything.
  • Ships a built-in NPS question ("How likely are you to recommend this game to a friend?"); other mods can register their own questions via the new SurveyRegistry API.
  • New config: surveyEnabled (default on) and surveyEmbedColor.
