Added new texture for Burnout Healer Accessory (cred for MrDoggy)

  CE cost and cooldown are now configurable via gamerules:
    /gamerule burnoutHealerCECost <amount>   (default: 200)
    /gamerule burnoutHealerCooldown <seconds> (default: 2, set to 0 to disable cooldown)
