🩺 Surgery Add-On
Surgery Add-On is a Minecraft Bedrock addon that introduces a complete organ-removal and transplantation system. Instead of mobs simply having health, they have an internal collection of organs that can be surgically removed, collected, traded between players, and transplanted into different bodies.
The addon is designed around experimentation: what happens if you replace your normal organs with organs from different mobs?
🧰 How to Start
The main tool is the Surgical Scissors.
Craft them at a crafting table using:
- 1 Lead
- 2 Iron Ingots
- 1 pair of Shears
- 1 Leather
Once you have the scissors, you can perform surgery on yourself, other players, and many mobs.
To operate on another mob, hold the Surgical Scissors and interact with it or use the scissors while looking at it.
Normally, a living target must be below 50% health before you can operate. Players can also allow surgery by sneaking. Creative players can operate without the normal health restriction.
To perform surgery on yourself, sneak while using the Surgical Scissors.
🫀 The Organ System
Bodies contain different organ slots, including:
- Heart
- Lungs
- Ribs
- Spine
- Muscles
- Stomach
- Intestines
- Liver
- Kidneys
- Spleen
- Appendix
There are several quality/tier variations, including Human, Animal, and Rotten organs.
The organs aren’t merely collectible items. They actually affect gameplay.
For example:
Heart: Your heart is essential for survival. Removing your heart causes severe damage and can quickly kill you. Extra heart capacity can provide regeneration.
Lungs: Missing lungs makes breathing extremely dangerous, especially underwater. Better lung combinations can provide improved underwater breathing.
Ribs: Ribs protect you from damage. Removing ribs makes incoming damage more dangerous, while having extra rib capacity can provide Resistance.
Spine: The spine affects movement. Without enough spine capacity, you become extremely slow and suffer Mining Fatigue.
Muscles: Missing muscles can cause Slowness and Weakness. Extra muscle capacity can provide Strength.
Stomach: The stomach controls digestion and hunger. Missing stomach capacity causes constant Hunger, while better stomachs can make food more effective.
Intestines: Intestines affect how efficiently you receive nutrition from food.
Liver: The liver affects how quickly toxic effects such as Poison and Wither wear off.
Kidneys: Missing kidneys can cause Poison and other negative effects. Better kidneys can help negative effects disappear faster.
Spleen: The spleen affects natural healing. A missing spleen can make healing less reliable, while additional spleen capacity can improve healing.
Appendix: The appendix has a special luck mechanic. Extra appendix capacity can increase the chance of receiving bonus loot from kills.
🧟 Mob-Specific Organs
This is where the addon becomes much more interesting.
Different mobs have different internal biology, meaning surgery can give you abilities normally associated with those creatures.
Drowned / Aquatic Mobs — Gills Gills provide Water Breathing while underwater, but they can cause Slowness on land.
Blaze — Blaze Core Provides Fire Resistance, but water damages you.
Enderman — Ender Heart Acts as a heart while giving you teleportation-related abilities. Water becomes dangerous.
Creeper — Volatile Gland One of the most unusual organs. Sneak for approximately three seconds to charge the gland and trigger an explosion without killing yourself. It has a cooldown, and fire can also activate it.
Rabbit — Rabbit Muscle Provides Jump Boost but comes with weaker attacks.
Horse — Horse Muscle Provides Speed but causes your hunger to drain faster.
Cow, Sheep, Goat, Llama, Camel and related animals — Rumen The Rumen functions as a specialized stomach. Plant-based food becomes much more effective, while meat is considerably less useful.
Wolves, Foxes, Cats and other carnivores — Carnivore Stomach The opposite specialization: meat becomes much more effective while plants provide considerably less nutrition.
Bat — Bat Ear Provides Night Vision, but bright daylight can sometimes cause Blindness.
Squid / Glow Squid — Ink Gland Attackers can receive Blindness when they hit you. The downside is Slowness while you’re outside water.
Iron Golem — Iron Muscle Provides Resistance, potentially reaching Resistance II with multiple Iron Muscles, but also causes Slowness.
Phantom — Wing Membrane Provides Slow Falling, but daylight causes Weakness.
Warden — Warden Heart One of the most powerful organs in the addon. It provides a major Health Boost and Strength, but comes with constant Darkness.
🩸 How Do You Get Organs?
There are two primary ways.
- Perform surgery
Find a compatible mob, weaken it enough to operate, then use your Surgical Scissors.
When you remove an organ, it becomes an actual inventory item. The item remembers information about its donor, allowing you to see where it came from.
You can then transplant that organ into another body.
- Kill mobs
When you kill a compatible mob, there is a 25% chance for one of its organs to drop fresh from the kill.
The dropped organ is randomly selected from that mob’s available organs.
Some organ combinations can also give you a chance to receive bonus loot when killing mobs.
🔄 Organ Transplants
The real gameplay loop is collecting organs and experimenting with combinations.
You can:
Remove → Collect → Store → Transplant → Experiment
For example, you could take an Ender Heart from an Enderman and use it as a replacement heart.
Or you could combine specialized organs to create a completely unusual build.
You aren’t limited to using organs from the same type of creature.
⚠️ Transplant Rejection
The addon also includes a rejection mechanic.
If you transplant an organ from another donor into a player, the player’s body can temporarily reject it.
During rejection, the player can experience Poison periodically.
Fresh organs taken directly from kills have a shorter rejection period, while organs obtained through other living donors can have a longer rejection period.
Eventually, the body accepts the transplant.
☠️ Surgery Has Consequences
This isn’t simply a cosmetic surgery system.
Taking out the wrong organ can kill a mob or player.
Mobs that require a heart can die if their heart is removed and not replaced. Completely emptying a mob’s available chest cavity can also kill it.
Players can likewise become extremely vulnerable when important organs are missing.
That means surgery becomes a risk-versus-reward mechanic rather than simply another crafting system.
🧪 Build Your Own Biology
The biggest feature of the addon is experimentation.
You can create different combinations of organs to specialize your character.
Want better underwater survival?
Use powerful lungs or Gills.
Want more protection?
Experiment with ribs and Iron Muscles.
Want speed?
Try Horse Muscle.
Want Night Vision?
Use Bat Ears.
Want to teleport?
Try an Ender Heart.
Want extreme power?
Experiment with a Warden Heart.
Want something completely ridiculous?
Combine several mob-specific organs and see what happens.
Surgery Add-On turns mobs into biological loot sources and transforms organ collection into an entirely new progression and experimentation system.
