I just discovered a bug involving the end portal, when you get back from the end to the overworld with the end portal your items get duplicated, sometimes cleared as well as the backpack's inventory. So pleas if you go to the end when comming back to the overworld don't use the end portal, you can use waystones, teleporters and other sutff. I honestly don't know how to fix it.
