Port of my mod to NeoForge 1.21.x. I don't plan on updating this version specifically because I want to keep it for 1.21.x NeoForge only.
Long story short, this mod lets you change your game window's title to whatever you'd like using Cloth Config API. The mod is enabled by default in the config, the window title is blank and there's also a keybind option to open config at any given time, which is not bound to anything by default, but you can change that!
