 - removed life binding creative tab.

 - renamed doomed effect to death.

 - milk, commands and totem of undying now cannot remove death effect.

 - prevented players who login getting the death effect to remove it by logging out.

 - data is now saved to the world, instead on players.

 - players will die from death effect when in other dimensions.

 - players with death effect will not take damage from other sources.
