Hotfix, some quick changes/fixes

• Fixed unarmed damage being applied to everything
• Lowered starting fist damage to default fist damage and increased damage increase per level
• Added option to disable fatigue completely, no more bleeding if you don't like it
• Fixed recovery time calculation which caused the player to need to wait longer each level instead of shorter,
  bleeding should be less bad now on higher levels
• Added option to change the damage multiplier used in the fist damage calculation
