Outline fixes

• Block outlines now recalculate properly when Block Xray is toggled
• Non-xray outlines ignore fully hidden underground blocks so visible blocks get priority
• Outline cache is cleared immediately when xray settings change

G-menu polish

• Existing goal count now sits left of the favorite star
• Favorite star is vertically centered in Add Items rows
• Add Items and side controls fit better on smaller screens
