When placing a skull without permission for spawning a Wither a dirt block will be placed. This is because there is a bug in CraftBukkit that does not cancel the BlockPlaceEvent properly for the new Skull block. So, to prevent duplicating Skulls I needed to place a dirt block in place of the Skull and then drop a Skull for pickup.
